Actually, Purpura Nephritis means that human body can’t produce urine through
Renal and metabolic waste and excess water can’t be discharged from body,
causing poisons. Modern medicine thinks that uremia is a series of complex
syndromes caused by body’s internal biochemical disorder after the loss of
kidney functions. But Purpura Nephritis is not an independent disease, which can
be called renal failure syndrome or HSPN in short.
Purpura Nephritis is a toxic effect. Because of chronic renal parenchyma
damage caused by various causes, metabolic wastes can’t be excreted and it can’t
to have the degradation of some endocrine. Uremic toxins include small molecular
nitrogen-containing substance, such as guanidine, urea, uric acid, amines and
indole class, etc, which are protein metabolisms; medium toxin substances,
including hormone with much blood retention, such as parathyroid hormone
peptide; large molecular toxic substance, such as glucagon, β2 microglobulin,
lysozyme, etc. About what mentioned above, some are toxic to human body and some
are harmless to human body when the density is normal. But when level in blood
get higher, toxicity occurs causing certain uremia symptoms. And Uremia occurs
with progressive disease course worsening to end stage.
Causes of uremia, which is common in foreign countries: Diabetic Nephropathy,
Hypertensive Nephropathy, Glomerulonephritis, Polycystic Kidney, etc. However,
in our country, the reasons are Primary Chronic Kidney, Obstructive Nephropathy,
Diabetic Nephropathy, Lupus Nephritis, Hypertensive Nephropathy, Polycystic
Kidney, etc.
Main Manifestations of Purpura Nephritis
Digestive Sympotom
Digestive symptom is the most common one. Almost every patient has the
symptom of inappetence, abdominal distension, nausea, vomit, sour regurgitation,
belching, etc. Reports in literature are that the incidence of gastrointestinal
ulcers symptoms is as high as 18% to 24%, which mainly is small vomiting or
melena.
Blood Systom Symptom
Uremic patients have different degrees of renal anemia, having skin and
mucous membrane bleeding tendency, performed as subcutaneous ecchymosis, gums,
subconjunctival hemorrhage, epistaxis, etc.
Cardiovascular System Symptom
Patients, who have uremia combined with renal hypertension, are as high as
80% to 90%, performed as hypertensive encephalopathy symptoms, such as headache,
dizziness, vomit, hyperspasmia, etc. The incidence of Uremic Pericarditis is as
high as 60% to 100%. Pericardial effusion can be found in 30% to 40% Uremic
Percarditis, mainly performed as precordial discomfort, choking sensation in
check and chest pain. The main cause of uremic death is cardiovascular system
changes. Uremic patients who die of vascular complications account for 64.3% of
the deaths.
Neuropsychiatric Symptom
When uremia occurs, the incidence of neuropsychiatric symptom is 13% to 86%,
which mainly is disturbance of consciousness, cranial nerve damage, muscle spasm
and epilepsy.
Respiratory System Symptom
The incidence of uremic lung infection is 50%, which is common in tracheitis,
bronchial pneumonia, purulent bronchitis.
Low Immune Function
The incidence of urinary tract infection is 44%, which is common in
gastrointestinal and skin infections.
Sexual Dysfunction
Men are accompanied by impotence and premature ejaculation. Women often have
menopause or menstrual disorders.
We should learn from this long period of misdiagnosis. People should think of
the possibility of Purpura Nephritis if there is unexplained chronic anemia,
gastrointestinal bleeding, epistaxis, skin congestion; unexplained hypertension,
especially among young patients, in which there is no effective treatment after
decompression; unexplained conscious disorders, symptomatic epilepsy; low or
high potassium or acidosis in patients which is difficult to correct. Then
people can take further examination and diagnosis.
